• Greetings from the RE Baker Health Office!

For the 2020-2021 school year, student medication drop off to the Nurse will look a little different. It is required to have the Permission Form for Prescribed Medications completed by the prescribing health care provider when bringing in the medication.

If your child has Asthma, Seizures, Diabetes or Life Threatening Allergies which require prescribed medication for emergencies or otherwise, the prescribing health provider must complete the appropriate plan.

Once these forms are obtained, call the Nurse to set up an appointment to bring in the medication. You must provide the most recent medication container or original box, with the identifying sticker from the pharmacy attached. Exceptions are over the counter medications that the health care provider has ordered. However, these must be in their original containers. Please do not cut or divide pills, the Nurse will do that in her office.

Any questions can be directed to the school at 479.254.5720.

    Have a great school year!
